ADDENDA AND NOTES

1. The following request was made and granted at the district's June 16, 2001 Board meeting. This request was made pursuant to section 3-1-2.

"I, Robert Ristinen hereby request approval to run a new type K copper service line from the meter on Apollo Drive generally south along my east property line for about 120 feet at which point it will connect to an existing Schedule 40 PVC service line now bringing water from the Apollo facility to my home. The existing line will be severed from connection to the Apollo system. This plan will permit me to use about 120 feet of existing service line which now runs under a concrete patio for about 50 feet of its length, and also runs under a concrete building footing."
